A rehab model where clinical excellence meets exceptional living
Harold’s family expected him to recuperate. They quietly hoped he would walk independently. What they never dreamt was that he would coach little league again, after an 18-year hiatus.
The transitional period after discharge is critical to future health and wellbeing. At Mills Pond we recognize
that each person needs and deserves a care plan that leads to their own ideal outcome.
That’s why each resident enjoys a dedicated trinity of doctor, nurse, and social worker who come together to formulate a custom rehab plan. That plan is then meticulously executed by our celebrated in-house physicians, therapists, and medical specialists.
Doting dining staff plans for special dietary needs; prepares gourmet dishes that taste like home cooked meals; and cater to group/private mealtime preferences.
Our concierge program steps in and steps up with any and all personal requests—from transportation to entertainment.
An enriching activities schedule makes the most, and safely pushes the boundaries, of each resident’s physical and mental abilities.
From admission through dismissal, our social workers assist each resident to ensure a seamless transition home.
